Parental Consent and Waiver of Liability
家长同意书与免责协议
I, the undersigned parent or legal guardian of the minor participant, hereby give my permission for my child to participate in the Pureking Badminton Academy Volunteer Program at the WanNeng Community Family Fun Sports Day held on April 26th, 2025 at Irvine Great Park.
本人作为未成年参与者的家长或合法监护人，特此同意本人子女参与 Pureking Badminton Academy 于 2025 年 4 月 26 日在尔湾大公园举行的“万能社群亲子趣味运动会”志愿服务活动。
I acknowledge and accept that there are inherent risks associated with participation in physical activities and events. I release and hold harmless Pureking Badminton Academy, its staff, volunteers, organizers, and affiliates from any and all claims, liabilities, or causes of action arising from any injury or accident occurring during or as a result of participation in this event.
本人了解并接受参与体育活动和公众活动存在一定风险。在此本人自愿承担相关风险，并同意免除 Pureking Badminton Academy、其工作人员、志愿者及组织者等一切与此次活动相关的伤害或事故责任。
I confirm that my child is in good health and physically able to participate. I understand that emergency medical treatment may be necessary in the event of injury, and I authorize the event organizers to obtain such treatment on my child's behalf if I cannot be reached.
本人确认我的孩子身体健康，具备参与活动的能力。如遇紧急情况，在无法联系到我的情况下，我授权主办方为我的孩子采取必要的医疗处理措施。
I understand that photos and videos may be taken at the event and used by Pureking Badminton Academy for promotional purposes. I grant permission for my child’s image to be used in such materials.
本人知晓活动期间可能会进行摄影和录像，并允许 Pureking Badminton Academy 将我孩子的肖像用于宣传资料中。
